COST: CERF eye testing - Dr Seth Koch, $33.00 per dog.
NO additional charge for CERF forms.
BAER hearing testing - Dr. Ellis Loew, Cornell University.
$45.00 for first dog, $35.00 for each additional dog, litters (pups under 6 months old on day of clinic) $40.00 for 1st four pups then $35.00 each additional pup
Cardiology testing - Dr. Nancy Morris, ACVIM Cardiology, Veterinary Emergency and Specialty Hospital, So. Deerfield, MA. Auscultation $35.00 per dog, echocardiograms (includes auscultation) $175.00 per dog, $155.00 per dog Holter monitoring (max. of 7/day), BNP CardioCare Blood test $65.00 per dog. Microchipping and CHIC DNA banking - Dr. Sheila Morrissey, Greenfield, MA.
CHIC DNA banking $5.00 for cheek swabs and $20.00 for blood samples (preferred method). $30 for microchipping (registration of chip NOT included)

TERMS: All fees in the form of checks or money orders must be prepaid to PVKC and included with the registration form. This fee is nonrefundable if you cancel your appointment within two days of the clinic, or if you do not show up for your appointment.
Appointments and time preferences will be scheduled on a first come first serve basis. We will do our best to accommodate your requests. If you register for multiple services, every effort will be made cluster your appointments.
